Mostly flange nuts are critical but some aren't.  In the electrical ground flange nuts are used in order to prevent the ground terminal from turning during the tightening process. As Chassis applications use washed nuts to commonize torques to vary greatly only when the nuts are tightened against the various chassis surfaces. The chassis attachment surfaces can be oversize hole and slots, cash surface, rough machined surfaces and many more.  Some joints do benefit from the use of a washer nut assembly but usually a loose washer with a nut is used.  This gives the bearing surface a more solid joint so that it can bear more pressure.

Flange nuts should be used for a more critical torque and nut and washer assembly are well fitted for low load sheet metal attachments. For big and large holes and slots a washer should be used but one should keep the load low otherwise the washer will "bell" into the slot. For different conditioned kind of surface a loose washer with a nut can work well but in such conditions a nut and washer assembly will also work only if the loads are low.
